- i. Eligibility criteria for pharmacists wishing to practice under a collaborative practice agreement and information outlining the required sections of a written CPA can be found in Section 465.1865, Florida Statutes.
- ii. A list of chronic health conditions for which a pharmacist can provide patient care services under a collaborative practice agreement can be located in Rule 64B16-31.007, F.A.C.
Requirements of CPA
- A written collaborative practice agreement must contain the following information:
- Name of the collaborating physician’s patient(s) for whom a pharmacist may provide services
- The chronic health condition(s) to be managed
- The chronic health conditions include:
- Anticoagulation management
- Arthritis
- Asthma
- Chronic obstructive pulmonary diseases
- Human immunodeficiency virus or acquired immune deficiency syndrome
- Hyperlipidemia
- Hypertension
- Nicotine dependence
- Obesity
- Opioid use disorder
- Type 2 diabetes
- A pharmacist may not modify or discontinue medicinal drugs prescribed by a health care practitioner with whom the pharmacist does not have a collaborative pharmacy practice agreement.
- A physician may not delegate the authority to initiate or prescribe a controlled substance as described in Section 893.03 or 21 USC Section 812 to a pharmacist
- Circumstances under which the pharmacist may order or perform and evaluate laboratory or clinical tests
- Conditions/events upon which the pharmacist must notify the collaborating physician and the manner and timeframe in which the notification must occur
- Beginning and ending dates for the CPA and termination procedures, including procedures for patient notification and medical record transfers
- A statement that the CPA may be terminated in writing by either party at any time
- The pharmacist entering into the collaborative practice agreement must submit a signed copy of the agreement to the board before the agreement may be implemented
- The pharmacist and collaborating physician must maintain a copy of the collaborative practice agreement at their practice site(s) and must make such agreement available to the department or board upon request
- If a required element of a written agreement is amended after initial submission to the board, the pharmacist and collaborating physician must keep a copy of the original agreement and the written amendment for proper documentation
- Written amendments do not need to be approved by the board
- The information highlighted above summarizes the minimum requirements to include in a written collaborative agreement per the Florida Statutes and Florida Board of Pharmacy. The example agreement included is not an approved template by the board. Final approval of a written CPA is at the discretion of the board.
Maintaining your CPA Certification on Your Florida License
- 8-hours of Board-Approved continuing education related to collaborative pharmacy practice each biennial licensure renewal
- Does not count towards general CE requirements for licensure renewal
- The pharmacist must submit proof of completion to the board when applying for licensure renewal
- If CPA certification is awarded less than 12 months from license renewal date, the 8-hour CE requirement is waived until the next licensure renewal cycle
